The Appointment Specialist is responsible for the safe and timely transportation of Group Home Residents to and from their medical appointments and to accurately relay and transmit details of the appointment between the resident’s medical providers and Berry Family Services Group Home and Nursing staff.  The Appointment Specialist reports to the Group Home Manager/Director

Responsibilities

1.Transport Group Home Residents to scheduled/standing medical and laboratory appointments in a timely manner and returning them to their respective day habilitation centers or group homes as required. Follow the standing schedule for ongoing lab appointments.
2.Relay pertinent information to doctors/nurse practitioners regarding resident’s appointment needs; including asking necessary questions, getting orders/refills, orders necessary for therapies and/or labs, DME supplies and standing orders signed.
3.Completing all HCS forms and returning them to the assigned case nurse in a timely manner .
4.E-mail the assigned case nurse and group home team after the appointment with information regarding the appointment by the end of that business day .
5.Attend and maintain initial/ongoing training for each resident with respect to special needs and medical issues/concerns.
6.Ensure the rights of the Individual, treating them with dignity and respect.
7.Complete all documentation required for Individuals and Company operations
8.Know and comply with all Company policies/guidelines per the policy and Employee Handbook/Guidebooks.
9.Conduct all matters in a professional and mature manner while representing the Company.
10.Model and reinforce all needs of Individuals and Group Home operations by “leading by example”.
11.Attend all in-services/training as scheduled to meet the needs of the Group Home, Individual, and the Company.  This role includes additional training on medical issues/concerns for the residents.
12.Communicate timely all vehicle needs to the Group Home Manager .  Ensure that there is always sufficient fuel in the vehicle, that registration is not expired, and that a toll tag is in place, and any maintenance needs, including warning messages on the vehicle are promptly reported and repaired.
13.If there are no medical schedules scheduled; assist the Group Home Manager and Group Home Manager assistants by:  Checking medication counts/expirations for group homes assigned by Management staff; checking MAR against medications in the homes, assisting with deliveries of groceries and other home supplies,
14.If necessary, fill in full or partial direct care shifts at any of the group homes.

Qualifications

Minimum age of 18.
Minimum education requirements:  HS Diploma or GED.  Prefer a candidate with 2 year's experience in direct care with the IDD population.
MUST be a licensed driver with a minimum of one year driving experience and a clean driving record
Must be able to pass a criminal background check and drug screen.
Must be able to speak, read, and write the English language.
