Responsibilities

The Registered Nurse (RN) is responsible to the Sr. Executive Director for the nursing needs of Berry Family Services. The RN works with the LVNs, Foster Providers and attendants to ensure the health and safety of each consumer. The focus of this position is to monitor the overall health care of the consumers, provide supervision and guidance to the LVNs and ensure the quality of all documentation.

  1. Adhere to the guidelines set forth by the Board of Nursing (BON) and the HCS Principles;
  2. Complete Initial Comprehensive Nursing Assessments on all new consumers;
  3. Complete Annual Physical Assessments, as needed;
  4. Complete Delegations when it is determined it is needed and ensures the delegated staff receives the appropriate training and skills are regularly monitored;
  5. Ensure tasks that cannot be delegated are completed by a licensed nurse;
  6. Ensure first dose of new medication is given by a nurse when medication administration is delegated;
  7. Complete Medication Training for all Direct Care Staff and Foster Providers, as needed;
  8. Make a determination of a Consumer’s Responsible Adult (CRA);
  9. Provide supervision of LVNs;
  10. Administer and monitor consumer’s medications;
  11. Monitor the consumers’ health data and information;
  12. Assist the consumers to secure emergency medical services;
  13. Make referrals for appropriate medical services;
  14. Perform health care procedures as ordered/prescribed by a physician/medical practitioner as required by standards of professional practice or law to be performed by licensed nursing personnel;
  15. Orders medications as needed for the Group Homes;
  16. Ensures a 72 hour supply of all medications are on hand at the Group Homes;
  17. Participate in Annual and Special Planning Conferences, as needed;
  18. Agrees to be on call after office hours, as needed to assist the consumers;
  19. Responds appropriately to all consumer crises;
  20. Complete monthly progress notes by the 3rd of the following month;
  21. Turn in billing to the Billing Department by the 3rd of each month;
  22. Participate in scheduled department and all staff meetings;
  23. Complete chart reviews of Medical Charts for Quality Assurance and to be survey ready at all times;
  24. Keep license current and in good standing.
  25. Other duties as assigned.

Qualifications

Certifications, Licenses, or Registrations:
  • Current certification from Texas Board of Nurses;
  • Current certification in CPR and First Aid;
  • Dependable transportation, valid driver's license and automobile liability insurance;
  • Recent TB test.
Experience:
  • Working knowledge of accepted nursing procedures for individuals with disabilities;
  • At least one year experience in home health;
  • At least one year of experience working with individuals with disabilities;
  • Familiarity with HCS regulations and contract standards of the program;
  • Must possess team-building skills in order to effectively work with others and provide quality services;
  • Must exhibit good relationship-building skills with consumers and their families; 
  • Computer literacy in word processing.
